The chapter opens with a cold, rainy night shift at the hospital. After the high-stakes surgery at the end of Chapter 3, Miya is physically exhausted but mentally hyper-aware. The central plot device of Chapter 4 is a "Code Violet"—a hospital code for a violent patient in the psych ward.

Here, we see Miya-chan not as a rookie, but as a diplomat. She disarms a delirious patient not with sedatives, but with a specific memory from her past: the scent of yuzu citrus. The patient, an elderly war veteran, mistakes her for his late daughter. Instead of correcting him, Miya plays along, calming him down instantly.
